All group leaders are encouraged to provide meeting attendees evaluation forms to complete and return.  This information is helpful in assessing speaker effectiveness, logistics, and impact of the program.  Leaders are encouraged to summarize and share this information with the Minnesota International Center.

July - December 2009 Meeting Report Form (word doc)
Group Leaders are asked to regularly report the meetings, topics, speakers and attendance of their group.  They are also asked to estimate the time they contributed in organizing group meetings.  Please download the form and either attach it to an email to Carol Steinberg or mail it to:

To order Great Decisions briefing books, teacher guides or DVDs, please download and complete this order form and send it to the Minnesota International Center (address above) with your payment. Materials relating to the topics are available at the beginning of each year. Depending on our inventory, you may receive ordered items in separate shipments. ST3 Tax Exempt Form (PDF)
Minnesota state sales tax (6.9%) applies to all orders.  If your organization is tax-exempt, MIC is required by law to have proof of your exemption certificate on file.  As of 2006, all tax-exempt organizations must complete a Minnesota Dept. of Revenue ST3 Certificate of Exemption form.  Please include this form with your payment, as we are unable to process your order without a completed ST3 form.  If we already have an ST3 form on file for your organization, you do not need to complete the form again.  To find out if MIC has a completed ST3 form for your organization, please email or call 612.625.4421.
